AWARDS
To Nominate an Individual or Organization for
an Award you must submit:
Nominations should be made for one of the
following categories:
Employer of the Year Award for
Transition
Presented to an employer in recognition of outstanding and ongoing
support in the employment of students with disabilities transitioning
from school to work.
Interagency Transition Council Award
Presented to an Interagency Transition Council that has demonstrated
passion, commitment and teamwork resulting in the successful transition
of students with disabilities to post-secondary settings.
Transition Visionary Award
Presented to an individual who has demonstrated a consistent history of
successful efforts to improve post-school outcomes for students with
disabilities through better transition practices.

AWARDS
To Nominate an Individual or Organization for
an Award you must submit:
Nominations should be made for one of the
following categories:
Excellence in AT Service Delivery Team
Award
The Service Delivery Team Award recognizes an AT Team, at the local or
statewide level, that involves two or more professionals or entities
that promote or facilitate the development of effective AT service
delivery. Factors considered include coordination of services,
networking and collaboration, innovation in development and
implementation, and lasting impact of the service provided on person(s)
served. This award honors excellence in the provision of AT services to
rural and/or other underserved people.
AT Visionary Award
The Visionary Award recognizes a leader in the field of Assistive
Technology in Georgia who has enhanced the field by demonstrating
vision, innovation, and/or initiative. This individual has done this
through advancing understanding, access, acquisition and/or quality
service delivery. This award was created by Tools for Life - Georgia’s
AT Act in recognition of outstanding achievement in the field.
AT Innovative Design
The Innovative Design Award recognizes outstanding design of a product,
building, space and/or program. This award was created by Tools for Life
- Georgia’s AT Act to promote creativity and encourage
universal design principles for equal access of all people.
Tools for Life and the Awards Nomination

three (3) pages, except in the case of the William E. Woodrick Case of

SERID AWARD CATEGORIES
Nominations should be made for one of
the following categories:
Outstanding Deaf/Hard of Hearing Citizen
Presented to a deaf/hard of hearing individual
who has exhibited outstanding leadership within
the deaf/hard of hearing community.
Distinguished Service Award
Presented to an individual who has demonstrated outstanding community
leadership, personal
achievement, or contributions to persons who are deaf/hard of hearing.
Outstanding Educator of the Year
Presented to an individual who exemplifies the highest standard of the
education profession
and who demonstrates extraordinary commitment to educational excellence
for the education
and welfare of persons who are deaf/hard of hearing.
William E. Woodrick Case of the Year
Presented in recognition of unique and/or outstanding efforts in the
rehabilitation of a person
who is deaf or hard of hearing in employment. Please follow the outline
below:

6. Summary and Justification for Selection of This Case
Please limit this recommendation to no more than six (6) typed,
double-spaced pages.
Attach the Informed Consent Form as a cover sheet.
Outstanding Interpreter
Presented to an interpreter in recognition of outstanding service to the
deaf/hard of hearing
community for higher efforts in making communication more accessible and
increasing awareness
of the need for better communication.
Employer of the Year
Presented to an employer in recognition of outstanding efforts in
recruiting and hiring persons
who are deaf/hard of hearing.
